#220 The Learning Curve of Minimally Invasive Transforaminal Lumbar Interbody Fusion Using Pedical Screws and a Translamina Facet Screw Hybrid Fixation

Abstract

Objective: To investigate the learning curve of the minimally invasive transforaminal lumbar interbody fusion ( MIS-TLIF ) using pedical screws and a translamina facet screw hybrid fixation.

Methods: A retrospective analysis was performed on the 48 patients who underwent MIS-TLIF using pedical screws and a translamina facet screw hybrid fixation in our hospital from October 2009 to July 2011. They were divided into chronological groups of twelve each: A-D. The operative time, intra-operative blood loss, the operative complications and the failure rate of translamina facet screw(TLFS) rate were recorded and compared among four groups.

Result: Mean follow- up duration was 16.3 months.The average operation time by group were: A, (139.17±18.32) min; B, (115.00±14.62)min; C,(110.83 ±11.04)min; D,(110.83±18.32)min. The average intra-operative blood loss by group were: A,(140.83±33.76) ml; B,( 83.33±28.39) ml; C,(69.17±25.03) ml; D,(64.17±25.75) ml. Both operation time and blood loss in group B,C,D were smaller and more stable than group A,P< 0.05, there is no statistical difference. Complications developed in three patients in group A, one in group B, and none in the other group. The failure TLFS number developed in two patients in group A, one in group B, and none in the other group.

Conclusion: The learning curve of the MIS-TLIF using pedical screws and a translamina facet screw hybrid fixation was about 12 cases. Working under straight view significantly lowered the learning curve without increasing the operation complications.
